Unclassified and Faculty Performance Evaluation Process

LSU Health - New Orleans is committed to a culture of employee success, continuous improvement, and ongoing performance feedback.  A critical component of these goals is the completion of our annual performance evaluation and success planning process.  
 
 

January 1, 2025 – December 31, 2025

Employee Evaluations must be completed with all parties’ acknowledgement and submitted to Human Resources via the online submission portal (Microsoft Forms) by the established due date of Monday, March 2, 2026.

1. Download the forms as PDFs to ensure accurate calculations. Do not use an internet browser.

2. Each section will auto-calculate. To get the overall evaluation rating on the last page, you must click the Calculate button. 

3. Signatures from both the employee and the manager are required. Employee signature does NOT indicate agreement with the rating reviewed, only that the evaluation has been discussed.

Performance Evaluation Portal Instructions

  1. Download the evaluation form.
  2. Fill out the evaluation form including the rating.
  3. Obtain supervisor, second level supervisor (if, applicable), and employees signatures.
  4. Access the Performance submission portal. The link for the portal is above.
  5. Answer the questions and upload the evaluation form, self evaluation form (if applicable), and any other supporting documents.
